Step 1: Complete the introductory requirements

In order to become a Vision Plan of America provider, you must:

Find a place to practice
Get liability insurance—you must have your own professional liability insurance of at least $1 million per occurrence/$3 million aggregate to become a VSP provider
Obtain your NPI number

Register with CAQH

Email: info@visionplanofamerica.com requesting optometrist enrollment information
1. They will email you a packet of information describing plan details and steps required for enrollment
Fill in and return:
1. Agreement form
2. Practice questionnaire
3. Copy of current state license
4. Current certificate of malpractice insurance
5. W9 tax form for each provider in the practice
You will receive a reply once credentialing is complete
