How to Become an EyeBenefits Provider
EyeBenefits is a discount vision plan used by groups and individuals nationwide. They offer an affordable vision insurance plan for individuals who do not have vision coverage through their employers. This insurance is accepted at many Walmart Vision Centers and other national and regional vision chains, as well as some private practices.
EyeBenefits offers a discount-only plan. Discounted agreements are listed within the agreement form you sign. EyeBenefits pays you after services are rendered. Additionally, your practice would be listed on EyeBenefits website for interested patients to locate.

Step 1: Complete the introductory requirements

In order to become an EyeBenefits provider, you must:
  1. Get your optometry license (see our guide that gives step-by-step instructions on how to get your optometry license in each state)
  2. Find a place to practice
  3. Get liability insurance—you must have your own professional liability insurance of at least $1 million per occurrence to become an EyeBenefits provider
  4. Obtain your NPI number
  5. Begin the Medicare enrollment process (see our 41 Step Guide to Become a Medicare Provider w/ Screenshots)
  6. Register with CAQH

Step 2: Complete EyeBenefits' plan requirements

  1. Visit the EyeBenefits website
  2. Click the link to “Sign Up to Become a Provider

  • This will open a PDF of the “Provider Network Partnership Agreement”
  • Fill out Agreement form

  1. Fax or email the Agreement form to the contact listed at the end of the form
Now, wait to receive confirmation from EyeBenefits, and begin the credentialing process for the next insurance on your list.
